The following open enrollment procedure shall apply to parents or guardians residing within the Los Altos School District who wish to enroll their child in a school in the district other than their school of residence.

2. COMPLETE FORM & RETURN IT TO THE DISTRICT OFFICE. Do NOT return this form to a school office.
