Immunization record submitted to Registrar must contain:
1. Child's first and last name
2. Child's birthdate
3. Immunization name and date of immunizations given
4. Doctor or clinic name
DTaP, DTP, Tdap, or Td - 5 doses
- 4 doses OK if one was given on or after 4th birthday
- 3 doses OK if one was given on or after 7th birthday
- For 7th-8th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday
Polio (IPV or OPV) - 4 doses
- 3 doses OK if one was given on or after 4th birthday
Hepatitis B - 3 doses
- Not required for 7th grade entry
Measles, Mumps, and Rubella (MMR) - 2 doses
- Both given on or after 1st birthday
Varicella (Chickenpox) - 2 doses
- If child has history of Chickenpox, only a medical exemption may be accepted in lieu of vaccination
Tdap - 1 dose
- Required at 7th grade entry/advancement
TB Requirement
- A TB Risk Assessment form (or a TB test if required after assessment) must be completed by a health professional in the U.S. for the following:
- New students (including TK/K)
- Returning students who have left Santa Clara County for 12 months or more, TB Risk Assessment form must be filled out and signed by a medical provider in the U.S. within 12 months of school registration
- Please note: TK-8 students transferring from another TK-8 Santa Clara County school are exempt from this requirement
If an immunization was not given due to medical reasons, a CAIR-ME (medical exemption) document is required to be submitted along with the immunization record. How to request a CAIR-ME document from your child's primary care physician.
