To Parents of Children Entering Kindergarten
in 2009-10 School Year:
To enter school children must meet the
California School Immunization requirements and the Santa Clara
County Tuberculosis (TB) School Mandate. Students will not be admitted
unless an immunization record is presented and immunizations and
TB tests are up-to-date.

This test must be completed within 18
months prior to kindergarten registration.
*Multiple puncture or prong tests (TB
Tine) will not be accepted
California law requires children entering school
to submit a record of a complete Health Examination. The "Report
of Health Examination for School Entry" form must be completed
within eighteen months prior to starting first grade. (Exam must
be given after March 1st for incoming kindergartners.) We ask
parents to submit this completed Health Examination form as part
of the kindergarten registration packet.
To meet the above three requirements,
have your child obtain the Immunizations, TB skin test, and Health
Exam at the same time. Information and forms are included in the
kindergarten registration materials. If you have any questions or
need additional forms, please contact your school.
If immunizations are against your personal
beliefs, you may come to the school office to sign an exemption
statement. If any immunizations were not given to your child due
to medical reasons, please bring a letter signed by your doctor.
As of July 1, 1999, a new state law went into
effect that requires students entering 7th grade to provide documentation
of the following immunizations:
- A series of three Hepatitis B immunizations.
For 11-15 year olds receiving adult (10mcg) dosage of Recombivox,
two doses, 4-6 months apart is an acceptable alternative schedule.
- A second Measles containing vaccine (usually
given as an MMR)
If your child has not yet received the Hepatitis
vaccine series, please start the series now so that all required
immunization records will be on file at your child’s current
elementary school by June 2003.
Students who have not received the Hepatitis
B series and their second Measles containing vaccine (MMR) before
the first day of 7th grade will be excluded from school and will
not be able to attend classes. We are alerting you to these immunization
requirements now in order to provide you with ample time to complete
the series prior to the first day of seventh grade.
Please bring a copy of your child’s
immunization record to your child’s current elementary school
office if you have not yet done so. This record should
indicate that your sixth grader has either completed or begun the
Hepatitis B series and received a second Measles containing vaccine
(MMR). Please indicate your child’s teacher and grade level
on this copy.
The law allows parents to elect exemptions to
immunization requirements based on personal beliefs or for medical
reasons. Exemption statements can be signed in the school office.
For children with medical exemptions, please bring a note signed
by your physician.
